Health & Wellness

  • Medical & Dental

  • OSF HealthCare provides comprehensive medical, dental, and prescription drug coverage.

    Eligibility
    Medical & Dental - Regular employment at 60 hours per pay period.
    Medical only - Regular employment at 32-59 hours per pay period.

    Effective the same day of the next month following your hire date (e.g. start date is 6/5, coverage date is 7/5)

  • Section 125 (Flexible Spending Accounts)

  • OSF HealthCare offers two types of flexible health spending accounts for setting aside tax-free dollars to help pay for eligible out-of-pocket expenses. If you elect a flexible spending account, you contribute to it before taxes in equal installments each pay period throughout the year.

    Our Unreimbursed Medical Spending Account is designed to help pay for medical, dental and vision expenses, up to a maximum of $2,600, while our Dependent Care Spending Account can help pay for day/child care expenses for children under age 13 and qualifying older dependents, up to a maximum of $5,000.

  • Vision

  • The vision plan may be purchased for Mission Partner and/or dependents and covers one (1) eye exam every 12 months with a co-pay and glasses (lenses and frames) every 24 months with a co-pay. In lieu of glasses, contact lenses may be purchased up to a certain allowance.

  • The longer you are a Mission Partner with OSF HealthCare, the more PTO you earn per pay period.

    Eligibility

  • Sick Time

  • Mission Partners earn 96 hours of sick time per year, up to a maximum of 240 hours. Sick time must be preceded with 16 hours of PTO; no pay if PTO is not available).

    Eligibility
    Regular employment at 32 hours per pay period.

    Effective Date
    Immediately. Mission Partners can use sick time after 90 days of employment.

Retirement

  • 401(k)

  • Mission Partners contribute a percentage of their salary (up to 90%, up to IRS limit) per pay period. OSF matches this contribution dollar for dollar up to the first 5% of your eligible pay.

    We also offer a catch-up contribution if you are 50 or older by year end (up to IRS limit; no employer match). An additional discretionary contribution may be made annually based on age and years of service.

    Eligibility
    Must be 21 years old and have completed 30 days of service.

    Effective Date
    Mission Partners may choose to enroll after two (2) weeks of employment or they will be auto-enrolled at 1% after 30 days of employment.

Education

  • Tuition Reimbursement

  • Reimbursement to a maximum of $3,500 per caledar year; pro-rated for part time Mission Partners.

    Eligibility
    Regular employment at 32 hours per pay period.

    Effective Date
    After 90 days of employment.

Insurance

  • OSF-Paid Life and AD&D Insurance

  • Life, accidental death and dismemberment insurance coverage equal to 1.5 times base annual earnings, to a maximum of $60,000.

    Eligibility
    Regular employment at 60 hours per pay period.

    Effective the same day of the next month following your hire date (e.g. start date is 6/5, coverage date is 7/5)

  • Voluntary Supplemental Life Insurance

  • Up to 5 times annual base salary with a guaranteed issue of $500,000/3x within 30 days of hire. Coverage may not exceed $1,000,000/5x. Spouse and dependent coverage is also available.

  • Voluntary Long Term Disability Insurance

  • Monthly disability benefit is 60% of base monthly earnings after 180 days of disability. Max monthly benefit is $10,000. Premiums based on age and salary.

  • Voluntary Critical Illness Insurance

  • Insurance that supplements your income with a lump sum payment in the event that you or a dependent is diagnosed with a critical illness, as defined by the policy. Various benefit levels are available.

  • Voluntary Accident Insurance

  • Insurance that assists your family in the event that you or a family member has an accident.
