Patient Experience Surveys
OSF Holy Family Medical Center utilizes Patient Experience Surveys to hear the opinions of our patients and families regarding the care and services we provide.
By completing the survey, you have the opportunity to recognize a nurse, doctor, technician, therapist, or any other member of our Holy Family Medical Center Team for his or her outstanding service. Perhaps you would like to comment on the cleanliness of our hospital or how much you enjoyed the meal you received.
You may receive a Patient Experience Survey from our staff as you are leaving our hospital, or you may receive a mailed or electronic survey.
We appreciate your time in completing this survey and for acknowledging our staff for their dedication and commitment to provide the best care possible for our patients and families.
The DAISY Award is for Extraordinary Nurses to honor the super-human work nurses do for patients and families every day.
The DAISY Foundation was founded in 1999 by the family of J. Patrick Barnes. Patrick died at the age of 33 from complications of ITP.
His family was completely awestruck by the care and compassion that they and Patrick were shown during his 8 week illness that they wanted to say THANK YOU to nurses.
Patrick’s wife, Tena, developed the DAISY acronym for Diseases Attacking the Immune System.
How Do I Recognize an Extraordinary Nurse?
If you have been the recipient of extraordinary care by a nurse and would like to nominate her or him please complete the DAISY Award Nomination Form.

What Happens After I Submit My Nomination Form?
Annually, a committee meets to review all the nomination forms received.
A ceremony and reception is held in the summer to honor the recipients. All nominees receive a DAISY pin and a copy of their nomination.
