OSF Holy Family Auxiliary Membership Application

Individual membership is open to any person interested in the objectives of the OSF Holy Family Auxiliary.

Fees

The annual dues of active members shall be $3.00 per year. Annual dues of supporting members shall be whatever amount the member may wish to contribute, or a minimum of $5.00.

Students, including high school and college, will not be assessed dues. Lifetime membership dues are a one-time payment of $100.00.
