The podiatry department is located in the new Rehabilitation and Specialty Clinics Wing on the north side of OSF Holy Family Medical Center.
To access this wing please use the driveway on the west side of OSF Holy Family Medical Center and continue around to the north side of the building.

What to Bring
Please provide a list of medications you are currently taking, including dosage and frequency of consumption, as well as your insurance or Medicare card(s) with billing address and policy numbers, and a photo ID.
We also ask that you bring any X-ray images or other pertinent medical records from a referring physician not with our facility.
