Instructions

To complete your virtual training, follow the steps outlined below.

1. Watch the training video below. Please allow two (2) hours to complete.
2. Download the Insulin Dose Calculator Sheet.
3. Download and complete the PDF evaluation.
4. Email your evaluation form to diabetescenter@osfhealthcare.org.
5. In your email with your evaluation, please let us know if you have questions following the training and would like to talk further by telephone to one of our diabetes educators. We would be happy to organize that time with you.
6. You will receive a reply with your certificate of completion. Note: if you do not hear back from a member of our team within five to seven (5-7) business days, please call us at (309) 624-2480.
