Faith Community Nursing Offering End of Life Planning Program

They are difficult conversations to have for any family – the end of life planning questions.

To help, the Faith Community Nursing program at OSF Saint Francis Medical Center is sponsoring the second annual Are You Ready? An Introduction to End of Life Planning program.

The FREE program will take place Thursday, August 7, 2014 at the First United Methodist Church Worship Center, 116 NE Perry Ave., Peoria from 1-4 PM.

Sessions offered include:

1-2 pm • End of Life & Health Care Issues: Patient rights & responsibilities, pain & symptom management, hospice benefits under Medicare, nursing home selection, coping with terminal illness and artificial hydration/nutrition.

2-3 pm • Financial & Legal Issues: Advance directives, wills/trusts, estate planning, guardianship, Medicare & Medicaid issues, important documents and family communication.

3-3:30 pm • Spiritual Aspects: Coping with terminal illness, spiritual support in times of crisis, funeral planning and religious views on end of life. Participants will be able to attend a break-out discussion with a faith community pastor from their religious background.

3:30-4 pm: Meet with a trained Advanced Care Facilitator to obtain an informational packet and set up an appointment to complete the forms.

Pastors from the Catholic, Methodist, Baptist and Lutheran faiths will be in attendance. Please RSVP before August 1 to Lisa Weber, RN at 309.824.8648 or with your faith community to make sure it is represented.
