OSF HealthCare is pleased to announce that providers with HSHS-affiliated Parkview Family Medicine have joined OSF Medical Group and will continue to offer primary care services in Streator beginning January 4, 2016.
Family Medicine physicians Glen Ricca, MD and Mark Wargo, MD and Internal Medicine physician Reshma Relwani, DO will remain in Streator, joined by returning Advance Practice Providers Joni Luckey, FNP and Patricia Hess, FNP.
“We’re pleased to have retained the talent and dedication of these well-liked and respected providers,” said Karen Brodbeck, OSF HealthCare Vice President of Public Relations. “Despite the changing health care climate in this region, patients who have relied upon their primary care services should not experience any disruption in care.”
Current patients with previously scheduled appointments beyond Jan. 4 will be seen on their designated date at a new location. New patients and appointments are being accepted.
“Patients will be seen in a new, expanded office location,” said Brodbeck. “Those affected will be notified by staff of the change.”
Currently located at 109 E. Elm Street in Streator, Parkview Family Practice will move to where St. Mary’s Hospital stands at 111 Spring St. As of Jan. 4 the new facility will become the OSF Center for Health - Streator
Drs. Ricca, Wargo, and Relwani, plus nurse practitioners Luckey and Hess, will be located on the third floor. The new practice, to be named OSF Medical Group - Center for Health - Streator, allows for an increase in office and exam room space in addition to a closer proximity to other on-site services.
An existing OSF Medical Group practice in the Kroger plaza at 2377 N. Bloomington St., Streator will continue to offer internal medicine, obstetrics and gynecology, pediatrics and orthopedic specialties at the current northside location.
In other provider and location updates relating to the upcoming transition, OSF officials confirmed:
- Dr. Riyaz Nomani, a full-time cardiologist with Prairie Heart Institute will be joining OSF. He will continue cardiology services but will move to a new location as well. The practice name effective Jan. 4 will become HeartCare Midwest - Streator. It will share the third floor of the new OSF Center for Health - Streator at 111 Spring St.
- Susan Pavlik, FNP-BC, currently practicing with HSHS Medical Group Family Medicine Wenona, will transition to OSF. She will remain at the 516 N. Chestnut St. location in Wenona. The new practice name will become OSF Medical Group - Wenona. It joins nearby OSF clinics in Minonk, Flanagan, Roanoke, El Paso and the Streator location.
- The HSHS facility on Ottawa’s south side, currently called St. Mary’s Health Care Ottawa, will remain open under OSF ownership following the Jan. 4 transition. It will continue offering family medicine, immediate care, occupational health and various on-site services. The new facility name will be OSF HealthCare - Ottawa South.
Practice names will be OSF Medical Group - Ottawa South and OSF PromptCare - Ottawa South, respectively. They will join existing clinics offered at the OSF Saint Elizabeth Medical Center campus on Ottawa’s east side. Additional on-site services of diagnostic imaging, laboratory and occupational health will be offered as an extension of departments of the same name at nearby OSF Saint Elizabeth.
To date, approximately 180 HSHS employees have accepted employment with OSF HealthCare and will continue offering care in Streator and neighboring communities.
