Galesburg, IL. – OSF St. Mary Medical Center was honored with an “A” Hospital Safety Score by The Leapfrog Group, an independent national nonprofit run by employers and other large purchasers of health benefits. The A score was awarded in the latest update to the Hospital Safety ScoreSM, the A, B, C, D or F scores assigned to U.S. hospitals based on preventable medical errors, injuries accidents, and infections. The Hospital Safety Score was compiled under the guidance of the nation’s leading experts on patient safety and is designed to give the public information they can use to protect themselves and their families.
“OSF St. Mary Medical Center is dedicated to improving safety for our patients. Many improvements in technology have improved patient safety.” said Carrie Hagen, Director of Quality, Safety & Risk Management.
In addition to Leapfrog, OSF St. Mary Medical Center has also been acknowledged by The Joint Commission, the nation’s leading authority on healthcare organizations, as a Top Performer on Key Quality Measures in the nation for delivering outstanding patient care.
OSF Healthcare implemented an electronic medical record that integrates care across the continuum from hospital, to offices, to home care, with access to other care providers outside of OSF. Physician order entry and bar code scanning of medications has also improved safety to patients. The patient nameband is scanned, medications are scanned, and are checked for duplications, interactions, correct identification, and correct timing prior to being administered to patients. If anything is incorrect, there is immediate notification prior to the patient receiving medication.
“The staff is also key to developing a culture of safety. Staff continuously communicate opportunities and assist with making improvements to keep our patients safe. The Leapfrog score recognizes the many efforts OSF St. Mary Medical Center to keep our patients safe.” said Roxanna Crosser, President of OSF St. Mary Medical Center.
“Hospitals like this that earn an A have demonstrated their commitment to their patients and their community,” said Leah Binder, president and CEO of The Leapfrog Group. “I congratulate OSF St. Mary Medical Center for its safety excellence, and look forward to the day when all hospitals will match this standard.”

Calculated under the guidance of The Leapfrog Group’s nine-member Blue Ribbon Expert Panel, the Hospital Safety Score uses 26 measures of publicly available hospital safety data to produce a single score representing a hospital’s overall capacity to keep patients safe from infections, injuries, and medical and medication errors.
