OSF St. Francis Hospital & Medical Group Earns Patient-Centered Medical Home Designation

Blue Cross Blue Shield of Michigan has designated OSF St. Francis Hospital & Medical Group – Family Practice Escanaba, Gladstone, and Powers and Internal Medicine as a Patient-Centered Medical Home practice for 2016, the third consecutive year. The designation period runs from July 1, 2016 through June 30, 2017.

This means we work together with patients to improve their care according to their personal health goals.

“The patient-centered medical home model is centered on the patient,” said Dr. Mark Povich. “We take a comprehensive approach to patient care helping patients meet their health goals and keeping track of all their specialist visits, test results and prescriptions.”

Blue Cross Blue Shield of Michigan has designated roughly 1,638 primary care practices — with 4,534 primary care doctors — as PCMH practices. The program is the largest of its kind in the country.

With the PCMH model, we create care teams that work with patients to keep them healthy and monitor their care on an ongoing basis. 

We coordinate patients’ health care using registries to track conditions and make sure they receive the care they need. We offer more access to the care team, coordinate other care (such as nutrition counseling) and help patients better manage conditions like asthma and diabetes. 

A recent analysis of Blue Cross claims data shows that PCMH-designated doctors are succeeding in more effectively managing patients’ care to keep them healthy and prevent complications.

As a Blue Cross PCMH-designated practice, we also participate with Blue Distinction Total Care. That means we provide patient-centered medical home care to patients who have a Blue Cross insurance plan from another state.
