OSF HealthCare Holy Family Medical Center Requests Community Input on Health Needs Survey

OSF HealthCare Holy Family Medical Center is requesting help from the local community to determine and prioritize health needs. A survey is now available, and residents can participate by filling it out online at welisten.at/Monmouth or by picking up a paper copy at the Strom Center, Inc., the Warren County Public Health Department, the Warren County Public Library, or the Warren County YMCA.

“Every community has different health care needs,” said Patty Luker, President of OSF HealthCare Holy Family Medical Center. “Through this survey, we hope to get input from a broad set of community members to help us focus our efforts on the challenges and opportunities unique to Monmouth and its surrounding areas.”

OSF HealthCare hospitals conduct community health needs assessments every three years and put together plans to meet the community needs that the assessments identify. After data is collected and each community's health needs are prioritized by greatest need, administrative and caregiver teams at OSF HealthCare work together to further support current community-based health programs and services, as well as starting new programs to help meet the needs for each community.

“Our last survey identified Healthy Behaviors – Active Living and Healthy Eating as one of the greatest opportunities for improvement in our community,” said Luker. “We took feedback from the survey and, through a collaborative approach, have provided two health events for the women of our community. Both of these events featured important screenings to maintain good health as well as a variety of stations to improve food choice. Additionally, OSF Holy Family Medical Center designed and implemented an innovative program to provide group activities and exercise for weight reduction.”
