(Kewanee, IL | June 24, 2019) –$10 sports physicals will be offered to all area athletes from Kewanee and surrounding communities Monday, July 1 and Tuesday, August 13 from 4 – 6 p.m. at OSF Medical Group-Kewanee, 1051 West South Street, Kewanee. Sports physicals are completed on a first come, first serve basis. No appointment needed.
Each student athlete is required to have a completed IHSA sports physical form that includes a parent/guardian signature. The IHSA form is available at www.ihsa.org/resources and also at the sports physical clinic. The form must be completed, signed by parent/guardian and brought to the sports physical. If an athlete sees a specialist, a medical release is required to be brought to the physical. Athletes wearing glasses or contacts are required to bring them to the sports physical.
The $10 sports physical fee will not be submitted to insurance and no credit or debit cards will be accepted. Exact change is encouraged.
A sports physical does not meet the requirement for an incoming 9th grade school physical. Please contact your student’s primary care physician to schedule a school physical as immunizations records are required.
