Applicants must demonstrate academic achievement with high school or college transcripts, volunteer or work history and health related activities.  Applicants must be planning to attend a post-secondary institution for a health career field of study.

To apply for either scholarship, interested students may download an application.  Completed applications must be returned to Susan Gorman at 901 Elm Pl Princeton, IL 61356, or emailed to gorman.susan@comcast.net, by 4 p.m. April 1, 2021.  Incomplete or late application packets will not be accepted.  Scholarship recipients will be notified of their award.  For more information, contact Susan Gorman at 815-878-5062.
