Chronic Care Management

Chronic Care Management is a program designed to improve the coordination of your health care with an emphasis on your overall well-being. This is achieved together through a partnership between the patient and their medical provider. 

By remaining involved in decisions regarding your health, health care and lifestyle, we can develop a stronger health relationship with you.

Helping you manage your chronic diseases.

Services include:

  • Scheduled “check-ins” with a nurse
  • Medication management
  • Assistance with community resources

Chronic Care Management is part of Medicare's objective to help people receive more goal-directed, holistic care planning and in-between visit care.

The program helps your provider give you a comprehensive care plan and enhanced engagement with your care. 

Throughout the month, your care team coordinator will call you to track your progress. 

If you have any new concerns or issues, you can address them with your care team at any time.

How do I qualify for the Chronic Care Management program?

Medicare patients with two or more chronic conditions are eligible for the program. Your provider will determine if your conditions qualify you.

Examples of common chronic conditions are: hypertension, hyperlipidemia, diabetes, hypothyroidism, chronic kidney disease, COPD, asthma, osteoporosis, ischemia heart disease, atrial fibrillation, stroke, anemia, cancer, depression, Alzheimer's disease and related dementia.

If you have two or more chronic conditions, your primary care provider will likely encourage you to participate. If you are interested, please contact your provider.

Will I be charged for Chronic Care Management?

You may be charged a nominal copay for this service. 

If you have coinsurance, supplemental insurance or Medicaid, the copay may be covered. Our office will bill Medicare for the Chronic Care Management services once a month.

Although you may not be in the office every month, your account will reflect this charge and you will be responsible for any copay that is not covered by your insurance(s).

How do I join the Chronic Care Management program?

We must obtain your consent to enroll you in this program. Our office will provide you with an agreement to sign. This agreement includes your authorization for electronic communications of medical information with other treating physicians or providers. You can cancel your Chronic Care Management agreement at any time. This can be done in writing at our office.
