Social Services

Our Social Services team serves as an advocate of the patient, focusing on the non-medical aspects of patient care. They assist patients and their families in adjusting to the social, psychological, financial, cultural and environmental impact of illness and/or hospitalization by:

  • Providing assistance in discharge planning for patients needing a level of care not provided in the hospital, such as assisted living, intermediate care or skilled nursing
  • Acquiring equipment or services the patient needs to return home safely (i.e., wheelchair, meals, home care)
  • Locating additional financial resources for which the patient may qualify to help cover the cost of hospitalization
  • Helping to ease stress caused by separation from home, work and family
  • Securing resources for patients who are terminally ill, chronically ill or are in need of rehabilitation
  • Preparing Advanced Directives
  • Evaluating, reviewing or completing a Living Will or Health Care Power of Attorney
  • Offering counseling, referrals and support to help patients and their families cope with illness and death
  • Obtaining community services and other area resources, as needed

The Social Services staff also facilitates a monthly adult grief support group for anyone in the community who has experienced the death of a loved one.  A social services staff member can be reached by calling (815) 431-5476.
