After reviewing the Non-Cognitive Performance Standards, please complete your application for the School of Radiography by following these four steps:
Step 1: Mail in Application Fee and Transcripts
Please mail a $50 nonrefundable application fee made payable to School of Radiography, along with all high school and college transcripts and ACT scores directly to the School of Radiography at the mailing address below.
Note: ACT scores are usually on high school transcripts, provided you took the exam at that time.

Note: If you sign the waiver on the reference form, you must have the referring person send the form directly to the School of Radiography mailing address below. If you elect not to sign the form, you may send in the completed references with your other materials.
