Your Treatment Team Includes:

Case Management

Our Case Managers help coordinate your comprehensive treatment plans.  The service helps to ensure families that their loved one is receiving the highest level of care – across the entire continuum of care.  They can even help with coordination between payers.  At any time during any rehabilitation treatment, questions or concerns can be forwarded to a case manager for immediate assistance.

Physical Therapy

Our Physical Therapists' (PT) goal is to optimize patient quality of life by restoring function and improving quality of movement.  Out PT team can help enhance patient independence.  Each therapist is skilled in the rehabilitation of musculoskeletal, neuromuscular and cardiopulmonary disorders and trained in a multitude of advanced therapeutic treatments.

Rehabilitation Nursing

Our rehabilitation nurse is a specialist who uses a variety of unique skills to make the most of the patient’s abilities. They will also work with you and your family or support system covering daily actives such as eating, grooming, bathing, dressing, bowel and bladder care, skin care, mobility, medications and overall safety.  In coordination with the rehabilitation team, nurses also work with patients to prepare them for a smooth transition.

Social Work

Our social worker will assist you in the adjustments to your life and planning for your future. They will provide ongoing support and counseling as you continue your journey. Your social worker can also assist in utilizing financial resources, filling out paperwork and accessing pre-authorization services. Your social worker is a great resource who can also provide you referrals for future care.

Occupational Therapy

Our Occupational Therapists' (OT) goal is to optimize patient quality of life by restoring function and improving quality of movement.  Our OT team can help enhance patient independence.   Each therapist is skilled in the rehabilitation of musculoskeletal, neuromuscular and cardiopulmonary disorders and trained in a multitude of advanced therapeutic treatments.

Speech –Language Pathology

Our Speech – Language Pathologists' goal is to assist patients with the ability to understand spoken and written communication and improve your ability to express your thoughts and needs. Along with communication, your therapist will work on your ability to swallow, as many of the same muscles are used. Working with a dietitian, meals that are easy and safe to be eaten will be planned.

Therapeutic Recreation (inpatient only)

Our Therapeutic Recreational Specialists' (TR) goal is to help improve patient functioning and provide recreational resources and opportunities. Re-entering the community is a major focus of therapeutic recreation. You and your family or care givers will enjoy therapy sessions in community settings. You’ll discover the activities your community has to offer and once you are discharged these activities can be carried on into your new lifestyle.

Medical Psychology

We are people just like you. We understand your condition is more than physical and can affect you as a person. That’s why we never forget the psychology and social aspects of your injury or illness. Our neuropsychologists provide treatment plans that help with attention, concentration, memory and problem solving.
