Social Services works to meet patient needs for economic, social, physical, and ministerial services.

• Coordinates discharge planning process including arranging adaptive equipment, home health and other care facility services and transportation needs.
• Maintains knowledge of community services, agencies, and resources available to patients.
• Assists in completing advanced directives.
• Interacts with patients, families and service organizations to promote continuity of care.
