Your core team will continue to be the professionals you already know and trust: your personal physician, an advanced practice nurse (APN) or physician assistant (PA), registered nurses and office assistants. When you need them, your care team can also include a behavioral health specialist, dietitian, pharmacist or social worker. A larger care team works closely together, ensuring you get the right care with the right caregiver.
Nurse Visits Add Convenience
To improve access and enable you to receive the right care at the right time, we're adding nurse visits for common concerns. Maybe you woke up with a sore throat or need a quick visit for a routine immunization. Our experienced nurses can address many routine health needs and provide preventive care. They also provide follow-up care and education for many conditions, including asthma, chronic obstructive pulmonary disorder (COPD), diabetes, heart failure and high cholesterol. Your OSF HealthCare team is eager to show you how we're making your care better, more convenient and well-coordinated.
