Learn what to expect and how to prepare for appointments at OSF HealthCare Cardiovascular Institute and its network of clinics. You’ll appreciate convenient locations throughout central and northern Illinois.

Making an Appointment

If you haven’t seen a cardiologist (heart doctor), you’ll need a referral from your primary care provider. Your provider will send the referral to your preferred OSF Cardiovascular Institute location, and the clinic will call you to schedule a visit.

After seeing us at least once, you can call your cardiologist’s office to request appointments.

Before Your First Appointment

Use OSF MyChart, your patient portal, to:

  • Answer questions about your health history
  • Complete preregistration forms
  • Designate a personal representative who your care team can communicate with about your health (if you choose)
  • Sign up for text notifications, such as appointment reminders

If you don’t complete the health history and preregistration forms before your appointment, arrive 30 minutes early to complete them in person.

For your convenience, you can print and complete the following forms before arriving at any of our OSF HealthCare Cardiovascular Institute locations:

What to Bring to Your First Visit

For registration, bring:

  • Health insurance card
  • Photo ID, such as a driver’s license
  • List of medications, vitamins and supplements you take and the doses

What to Expect

A nurse or medical office assistant will:

  • Measure your weight, blood pressure and pulse
  • Review your medications and allergies

Then, a doctor or advanced practice provider will:

  • Review your health history
  • Discuss your symptoms
  • Perform a physical exam
  • Explain their findings
  • Recommend tests or treatments

If you need additional care, a staff member will schedule it. Your primary care provider will receive a copy of your care plan.

How to Cancel an Appointment

If possible, call at least 24 hours in advance to cancel your appointment. By doing so, you’ll allow someone else the opportunity to use your appointment time.
