Swing Bed Program

OSF Saint Paul Medical Center is proud to offer our swing bed program - skilled nursing for those in need of transitional care before returning home. This program eliminates the inconvenience and worry of being hospitalized away from your family and support system by providing swing beds.

Our Swing Bed Program allows patients who no longer need acute care to move to another level of care in a hospital closer to home before being discharged. It’s all part of making you as comfortable as possible while creating opportunities for your continued path to a full recovery.

The length of your stay in the program will depend upon your condition and individualized treatment program. The Swing Bed Program helps make the transition from the hospital to home safer and easier.

For example, if you live in Mendota and have a hip replacement at OSF HealthCare Saint Elizabeth Medical Center in Ottawa, you can be transferred to the Swing Bed Program at OSF Saint Paul to be closer to home once you are released from OSF Saint Elizabeth.

Sharing_Health_I-80_swing_beds_20180130-1207.jpg By transitioning from an OSF HealthCare facility for acute care to the OSF Swing Bed Program, patients have the advantage of receiving the same level of expert care they know and trust. Additionally, OSF HealthCare uses one medical record system. This gives the Swing Bed Program care teams the ability to review each patient’s medical history to optimize their treatment plan for their individual needs and help them heal as quickly and safely as possible.

Planning an elective surgery or wondering how you’ll transition home following a hospital stay? Ask your provider or discharge planner if the OSF Swing Bed Program is right for you.  For more information call OSF HealthCare Saint Paul Medical Center at (815) 539-7461 or contact OSF Saint Paul Case Management at (815) 539-1628.
