Our mission is to provide referral services to our patients and help to connect them to an array of social services available in the community.

Community Resource Center

We take a holistic approach to medical problems and aim to address unmet social needs of patients in the emergency department or directly at the Community Resource Center (CRC).

No matter what might be wrong, the Community Resource Center at OSF Heart of Mary Medical Center will connect you to organizations that might be able to help.

We’ll work through just about any issue you may have, health-related or not.

Faith in Action in Urbana 

Faith in Action volunteers at OSF HealthCare Heart of Mary Medical Center provide outreach in Champaign County by helping seniors who are 55 years and older with long-term health needs live a safe and independent lifestyle.

We are a network of volunteers providing practical, emotional, and spiritual assistance for seniors so they can live a safe and independent lifestyle.

Services include but are not limited to: 

  • Assistance with shopping/errands
  • Health education and exercise classes
  • Home and telephone visits
  • Seasonal events
  • S.H.I.P/ Medicare assistance
  • Social events

Senior Health Insurance Program (SHIP) 

The Senior Health Insurance Program (SHIP) is dedicated  to educating you and your caregiver about Medicare and the available insurance options. 

SHIP is a free counseling service provided by the Illinois Department of Aging. It is not an insurance plan, but trained counselors who can provide one-on-one help in navigating Medicare. 

The SHIP counselors can answer questions about Medicare, Medicare supplement insurance, advantage plans, HMOs, PPOs, PFFs, prescription drug coverage, long-term insurance, claims and appeals and beneficiary rights and guarantees.
