At OSF HealthCare Divine Mercy Continuing Care Hospital, we value patients, families and caregivers as essential voices in shaping every step of the care experience. Your perspective can directly inform how we strengthen safety, elevate our service and continuously refine the care journey for every patient and family we serve.

What is the Patient & Family Advisory Council?

The Patient & Family Advisory Council (PFAC) brings together patients, family members and caregivers to work alongside hospital leaders and staff to share real experiences and ideas. This partnership helps ensure care decisions are grounded in what matters most to patients and families.

What Council Members Do

Your voice helps shape real change. Council members share their experiences to help guide meaningful improvements in care. This may include:

  • Offering feedback on hospital programs and services
  • Sharing perspectives on the care experience
  • Participating in discussions with staff and leadership
  • Helping identify opportunities to improve patient and family support
  • Collaborating on ideas that strengthen safety and care quality

The PFAC is one way we bring that commitment to life. Your insight directly supports our mission to provide high-quality, compassionate care.

Who Should Apply

Your experience is what matters most, so you don’t need a health care background to join the PFAC. We’re looking for people who:

  • Have personal experience with care at OSF Divine Mercy Continuing Care Hospital within the past two years
  • Are willing to share honest feedback and ideas
  • Enjoy collaborating with others in their community

Time Commitment and Term Details

The PFAC meets four times per year, either in person or virtually. Meetings typically last no more than two hours.

Members commit to:

  • A one-year term
  • Attending at least three of the four annual meetings
  • Actively participating and sharing feedback during discussions

How to Apply

We’d love to hear from you. Selected applicants will be contacted to participate in a brief interview. Final selections will be based on interest, experience and the goal of building a council that represents the patients, families and communities we serve.
