Community Resource Center

We take a holistic approach to medical problems and aim to address unmet social needs of patients in the emergency department or directly at the Community Resource Center (CRC).

No matter what might be wrong, the CRC at OSF HealthCare Sacred Heart Medical Center – Urbana will connect you to organizations that might be able to help.

We’ll work through just about any issue you may have, whether it's health related or not.

Faith in Action in Urbana

Faith in Action volunteers at OSF HealthCare Sacred Heart Medical Center – Urbana provide outreach in Champaign County to seniors who are 55 years and older with long-term health needs.

We provide practical, emotional and spiritual assistance so you can live a safe and independent lifestyle.

Services include but are not limited to:

  • Assistance with shopping/errands
  • Health education and exercise classes
  • Home and telephone visits
  • Seasonal events
  • Senior Health Insurance Program/Medicare assistance
  • Social events

Senior Health Insurance Program (SHIP)

The Senior Health Insurance Program (SHIP) is dedicated to educating you and your caregiver about Medicare and the available insurance options.

SHIP is a free counseling service provided by the Illinois Department of Aging. It is not an insurance plan. We connect you with trained counselors who can provide one-on-one help navigating Medicare.

The SHIP counselors can answer questions about Medicare, Medicare supplement insurance, advantage plans, HMOs, PPOs, PFFs, prescription drug coverage, long-term insurance, claims and appeals and beneficiary rights and guarantees.
