OSF Healthy Joints Program
The OSF Healthy Joints program is a collaborative effort between Physical Therapy, the Center for Healthy Lifestyles, Behavioral Health and the Weight Management Center of OSF HealthCare St. Joseph Medical Center.
The goal is to help you optimize your health and lower your body weight to preserve your joints or reduce the risk of complications that may occur when you undergo elective joint replacement surgery.
The program is specifically designed for people whose weight is impacting their joint pain.
How does it work?
As a participant in the 20-week OSF Healthy Joints program, you will work through four Principles of Success that include components of physical therapy and exercise physiology, the Health Management Resources (HMR) meal replacement program and behavioral health.
OSF HealthCare knows the weight loss journey is not easy and support from friends and family makes all the difference. We encourage you to engage those in your life who are important to your success.
The Principles of Success
The four Principles of Success include:
Embrace Movement – OSF HealthCare therapists and exercise physiologists will help to identify imbalances in your body and address pain and problems. In addition to traditional physical therapy evaluation, the program uses the METHOD CRA test to determine your metabolic prime, anaerobic and peak heart rate zones to maximize your body’s ability to burn fat for fuel.
Information from this specialized test is used in the development of your personalized movement program to meet your unique physical needs and improve your health at the cellular level.
The Center for Healthy Lifestyles offers an affordable membership option where you can exercise in a comfortable and safe environment.
Eat with a Purpose – Using the scientifically proven HMR weight-loss program, this principle incorporates two phases.
Phase 1 introduces you to the HMR mealreplacement program and related support and education.
Phase 2 expands on your nutritional options. Following completion of the HMR program, you may choose to take advantage of nutritional programs available through the Center for Healthy Lifestyles to continue your journey to health.
Redefine Perceptions – Working with Behavioral Health, you will begin to change your relationship with food through personal evaluation and support sessions. This information is reinforced by other members of your Healthy Joints team.
Engage Your Family – Working with members of all the supporting teams, you and your family embrace the lifestyle change through education, commitment and involvement in your shared success.
Throughout each of the Principles of Success, a key family member – who is considered a coach – is encouraged to participate by attending orientation sessions, extending support and accountability, and communicating with members of the OSF Healthy Joints team.
Healthy Joints Navigator
A patient navigator from the Center for Healthy Lifestyles is there to guide you and your coach through the entire circle of care provided through the OSF Healthy Joints program.
The navigator will work with you to obtain required referral information from providers, when necessary.
Why the need?
OSF HealthCare St. Joseph Medical Center cares deeply about your well-being.
To provide the best outcome from your elective joint replacement surgery, and to decrease your risk of complications, OSF St. Joseph no longer accepts referrals for elective joint replacement for people with a body mass index (BMI) greater than or equal to 40.
Individuals with a high BMI will be referred for weight loss and health optimization, and OSF Healthy Joints is pleased to provide a solution to support their journey. Following health optimization, OSF St. Joseph will be pleased to schedule elective surgery.
