Jason Rodeghero,

President

Jason Rodeghero

Jason Rodeghero has served as President of OSF Home Care and Rehabilitation Services since 2020.

He joined OSF HealthCare in 2007 as a physical therapist. He served in different leadership positions for the OSF Eastern Region before moving into a Ministry leadership role in 2015.

As President, Jason oversees home health, hospice (including the OSF Richard L. Owens Hospice Home), home infusion pharmacy, home medical equipment, post-acute skilled nursing facility practices, outpatient palliative care, and all therapy/rehabilitation services.

Jason received his bachelor's degree from Truman State in Kirksville, Missouri; master's degree from Bradley University; PhD from Rocky Mountain University of Health Professions in Provo, Utah; and Master of Business Administration from South College in Knoxville, Tennessee.


Sarah Overton,

Chief Nursing Officer/Vice President of Clinical Services

Sarah Overton 

This role includes responsibility for nursing in all multi-specialty settings for all OSF service regions as well as responsibility for the strategic initiatives to ensure consistency for clinical practices. 

Sarah has served OSF Healthcare System for in various roles 15 years.  

Sarah has been a nurse for 16 years; completing her RN-ADN through Eastern Iowa Community College, obtaining her BSN through UIC, and her masters though University of Illinois at Chicago,  in Peoria.

 

 


J.J. Guedet,

Vice President of Business Development and Strategy

J.J. Guedet 

J.J. Guedet has been serving OSF as Vice President of Administration & Strategy – Home Care since 2013.  In his role, J.J. is accountable for the administrative services that support clinical operations and developing strategic relationships with internal and external stakeholders. 

J.J. serves as the executive leader of Skilled Nursing Facility Services, Home Medical Equipment & Infusion Pharmacy, Supportive Care, and the Centers of Expertise for Home Care & Rehabilitation. 

He works closely with the Home Care and Rehabilitation Vice Presidents of clinical operations as well as other Ministry executive leaders. 

In partnership with operational leadership, J.J. identifies improvement opportunities and focuses on implementing strategic initiatives that optimize performance. 

J.J. has 12 years of experience in healthcare, all with OSF Home Care.  Prior to working with OSF he worked as a Business Intelligence consultant with clients in other industries.  J.J. is a graduate of Taylor University, with a bachelor’s degree in Accounting and Information Systems.


Lena Nilsson,

Vice President of Clinical Operations

Lena Nilsson 

Lena serves as the executive leader responsible for defining and executing strategies related to the care provided by OSF Home Care clinicians in Home Health and Hospice.

Prior to joining OSF, Lena served as the general manager of a large hospice agency in Sacramento, California. She successfully led many projects, including the development of various strategic partnerships with both acute and post-acute care facilities.

Lena received a Bachelor of Arts in political science from the University of California Los Angeles (UCLA).

She went on to earn a Master Business Administration in health care management and organizational leadership from Johns Hopkins University in Baltimore, Maryland. 


Dawn Lemmert,

Director of Medical Services

Dawn Lemmert 

Dawn Lemmert, MD, serves as the Director of Medical Services for OSF Home Care. In this position, Dr. Lemmert works closely with leadership to oversee the medical care provided to patients across Home Care Services.
 
Dr. Lemmert has been a Mission Partner since 2002 when she joined the OSF Pontiac family practice.

In 2011, she transitioned to the OSF Morton clinic. She then became the lead provider for the Skilled Nursing Facility Practice in 2014. This work has included collaborating closely with network facilities to improve quality and safety in the homes and establish best practices.

In addition, Dr. Lemmert actively teaches hospice and palliative care fellows and has been a member of the fellowship program faculty since its inception.
 
Dr. Lemmert received her Bachelor of Science in biology from Southern Illinois University at Carbondale and her doctorate degree from the University of Illinois. Her family medicine training was completed at Memorial Hospital of South Bend, Indiana.
