What is OSF MyChart?

OSF MyChart gives you direct online access to some of your Electronic Medical Record (EMR), where your health information is securely stored and password protected.

With OSF MyChart, you can easily access your medications, health summaries, lab results and OSF appointments, as well as communicate with your provider. You can also track your inpatient stays in real time at most OSF HealthCare hospitals.

Best of all, this information is available to you 24/7 at osfmychart.org or with the MyChart mobile app available for Apple and Android devices.

What you can do with OSF MyChart

Many things that used to require a call or visit to your provider can be handled with a few keystrokes or by touching the screen on your mobile device. Here are some of the features you may find most useful.

  • View your medical history

    • Review your medications, immunizations, allergies and medical history.
    • Receive test results online – no waiting for a phone call or letter.
    • Review health education topics and After Visit Summaries® from your health care provider.
  • Contact your care team

    • Communicating with your doctor and care team is as simple as sending an email – but even more secure when you use OSF MyChart.
    • Save time by requesting prescription renewals through OSF MyChart when it’s convenient for you – 24/7.
    • Message questions or updates to your care team on your timetable.
    • View notes written by your care team.
  • Schedule and manage your appointments

    • Schedule appointments for primary care, flu shots, screening exams and more.
    • View real-time availability and select the time that’s best for you.
    • Receive appointment reminders.
    • View details of your past and upcoming appointments in one place.
    • Speed up the check-in process for appointments.

    • Log in to your OSF MyChart account.
    • Click the Visits icon at the top of the screen.
    • Click the Schedule An Appointment button.
    • Select a specific provider or choose the type of appointment you wish to schedule.
    • Follow the prompts and set your appointment date and time.
  • Access Your Family's Records

  • Link your young children’s OSF MyChart accounts to yours for convenient access to appointments, immunization records, growth charts and more. This is called proxy access. You can also use this feature to grant another adult access to your medical records.

    HOW TO: Access your family's records

    • Click the Menu icon at the top of the screen.
    • Within the Menu, go to the Communication section and click on Request Family Access.
    • Click the link to “Submit request to access a minor’s record” to request records for children under 12 years old.
    • Click the link to “Friends and Family Access” to grant or revoke access to your record.
    • Download and print out an Adult Proxy Form that you must complete to request access to the records of an adult who is incapacitated or unable to make decisions. Present the completed form to the patient’s provider or the hospital.

    To access records for your children 12-17 years old, contact your provider directly to obtain forms that must be filled out and signed.

  • Access information from your hospital visits

  • At OSF HealthCare, we want you to be actively engaged and participate in your treatment – including when you’re in the hospital. When you become an inpatient at most OSF HealthCare hospitals, you can use your OSF MyChart account to access information in real time.

Additional features

Make the most of your health care experience by taking advantage of these advanced features in OSF MyChart. Many were designed with your convenience in mind.

  • Convenient and early check-in

  • Use OSF MyChart to speed up the check-in process for appointments. You can eliminate some waiting-room procedures before you even leave home.

    The eCheck-In feature is available at most OSF HealthCare locations, starting 24 hours before your scheduled appointment. It can be used for your provider appointments and hospital outpatient appointments, such as rehab, diagnostic tests and more. eCheck-In is required for all video visits.

    Use eCheck-In to do the following:

    • Confirm your appointment.
    • Confirm or update current medications, allergies, health issues and medical history.
    • Answer general health questions and e-sign documents.
    • Verify or update your insurance and other personal information.
    • Pay your copay.
    • Select your preferred pharmacy.

    HOW TO: Use eCheck-In

    • Log in to your OSF MyChart account.
    • Click on the Visits icon at the top of the screen.
    • Select Appointments and Visits.
    •  Scroll to your upcoming appointment and click the eCheck-In button. (Note: The eCheck-In button will not appear until the 24-hour window before your appointment.)
    • When you arrive at your in-person appointment, you still need to tell the check-in desk you have arrived. There may be additional paperwork to be completed upon your arrival.
  • Review care team notes

  • As an OSF patient, you can now read the notes some members of your care team write following an appointment. It’s all secure and online through your OSF MyChart account.

    Reading your notes can help you:

    • Remember what was discussed with your care team.
    • Feel empowered to make health care decisions.
    • Partner with your care team for better care.
    • Make sure your information is accurate so your care is safer.
    • Share your information with a loved one or anyone you choose.

    HOW TO: Review care team notes

    • Log in to your OSF MyChart account.
    • Click on the Visits icon at the top of the screen.
    • Select Appointments and Visits. 
    • Click the View Notes link on a past visit.
  • Share your record

  • With OSF MyChart, not only can you access and read your medical record at any time, you can also share it as needed with your other health care providers. Use the Share Everywhere feature to grant them temporary access to your records.

    HOW TO: Share your record

    • Log in to your OSF MyChart account.
    • A Share Everywhere notice may appear near the top of your screen, but you can easily find it in the Menu under Sharing.
    • Click the Learn More button.
    • Type in the name of the provider to whom you are granting access.
    • Click the Request Share Code button. 
    • You will receive a code they can use one time to view your record.
  • Find urgent care now

  • If you need immediate care for an injury or illness, you can review walk-in waiting times or secure a same-day appointment at an OSF urgent care clinic near you. Use this feature to see clinic locations and decide whether to add yourself to the walk-in queue at an OSF PromptCare or schedule an appointment at OSF OnCall Urgent Care. (You can also make appointments at these facilities for school and sports physicals, employer drug and alcohol testing, seasonal flu shots and more.)

    HOW TO: View walk-in waiting times or make an appointment

     

    • Log in to your OSF MyChart account.
    • Click the Menu icon at the top of the screen.
    • Within the menu, go to the Find Care section and click on Find Urgent Care.
    • Select the reason for your visit.
    • Select the location you wish to visit.
    • If you choose an OSF Prompt Care, click on Get In Line, then provide your estimated arrival time.
    • If you choose an OSF OnCall Urgent Care, choose an available time for your appointment.
  • Video visits

  • OSF offers an alternative to traditional in-person appointments. Video visits, sometimes called telehealth or virtual visits, are an easy and convenient way to stay connected to health care, wherever you are.

    • Log in to your OSF MyChart account.
    • Click on the Visits icon at the top of the screen.
    • On the Appointments and Visits page, click the Schedule an Appointment button.
    • Click the Video Visit section.
    • Select your provider and make your appointment.
