Thank a Caregiver

OSF HealthCare strives to provide exceptional care to every patient, every time. But often, it's that added touch or simple act of caring
that sets a visit apart.  For those moments when your caregiver goes above and beyond, help us recognize those individuals for their extraordinary dedication.

Did you have an outstanding patient experience?

If so, allow us to honor the OSF Mission Partner who provided your compassionate care and service. Submit their name for an award using one of the links below.

About the DAISY Award

The DAISY Foundation was established in 2000 by the family of J. Patrick Barnes who died from complications of the auto-immune disease ITP at the age of 33. DAISY is an acronym for "diseases attacking the immune system." During Pat's eight-week hospitalization, his family was awestruck by the care and compassion his nurses provided not only to Pat, but to everyone in his family.

One of the goals they set in creating a foundation in Pat's memory was to recognize extraordinary nurses everywhere who make an enormous difference in the lives of so many people by the super-human work they do everyday.

DAISY Award honorees exemplify a remarkable patient experience. These nurses consistently demonstrate excellence through their clinical expertise and extraordinary compassionate care. They are recognized as outstanding role models within the communities we serve.

Patients, visitors, and OSF Mission Partners may nominate a nurse by selecting the appropriate link above.

About the Sunflower Award

Based on the success of the DAISY Award program and its positive impact on the nursing profession, OSF HealthCare created a recognition program of its own. The Sunflower Award was designed to award exceptional caregivers from all disciplines.

A recognition program offered exclusively through OSF HealthCare, the Sunflower Award honors those non-nursing staff members who exemplify a remarkable patient experience.  These individuals consistently demonstrate excellence through their expertise and extraordinary compassionate care.  They, too, are recognized as outstanding role models within the communities we serve.

Patients, visitors, and OSF Mission Partners may nominate a non-nursing staff member by selecting the appropriate link above.
